Accessory breast tissue is an additional breast tissue which gets developed outside the normal breast area. This area is commonly armpit or axilla. Accessory or accessory breast occurs when some breast tissue remains along the embryonic milk line, which gets extended from the armpit towards the groin during early development.Â
This tissue may become more noticeable after hormonal changes associated with pregnancy and breastfeeding. This is a case study of 38-year-old woman from Kandivali, Mumbai. She developed bilateral accessory breast tissue in both armpits after her pregnancy. She was struggling with it since last three to four years.Â
In this case study, Dr. Vishal Patel performed bilateral axillary breast removal using liposuction method. We will cover the patient’s condition, the surgical approach, post-operative care, and the results using before-after photographs of the surgery. So let us dive deep.

Condition Before Accessory Breast Removal Surgery - Findings by Dr. Vishal Patel
The woman came to Recon Plastic Surgery & Physiotherapy Clinic to discuss about her bilateral accessory breast tissue in her both armpit in Oct 2026. Patient mentioned that she noticed soft tissue fullness on both sides after just few years of pregnancy.Â
By Oct 2026, the additional tissue now started affecting the natural contour of the armpit and is a visibility concern for the patient.Â
During the consultation at his plastic surgery clinic in Malad, Mumbai, Dr. Vishal Patel examined the fullness in both armpits. He assessed the location and distribution of additional tissue. He also considered how the tissue has contributed to underarm bulges.Â
The patient’s history of developing the condition after pregnancy and the presence of fullness in both axillary regions were key details in the clinical evaluation.Â
Based on the patient’s condition and underarm contour, Dr. Vishal Patel agreed with performing bilateral accessory breast removal using liposuction without skin excision.Â
Step 1 : Precise Marking of Accessory breasts
Before starting the operation for accessory breast removal, Dr. Vishal Patel carefully marked patient’s underarm region to identify the areas affected by accessory breast tissues.Â
Below are the pre-operative photograph and surgical marking done in purple ink to outline prominent tissue bulge.
This marking will help the surgeon to establish the boundaries of excess tissue. Dr. Vishal Patel also assessed distribution of the fullness and identified extent of the bulge during this stage. It is important to do careful pre-operative marking as each patient’s underarm contour are different.
Step 2 : Fat removal with Liposuction
Once the markings are established, Dr. Vishal Patel and his surgical team performed suction-based liposuction to remove excessive fatty tissues from both underarms. You can see below photographs where Dr. Vishal Patel is using special liposuction cannula to access and remove targeted fatty tissues with small incisions.Â
Dr. Vishal Patel used this liposuction technique because of patient’s condition and the ability to achieve natural contour. During the liposuction, general anesthesia was used for the procedure, which took approximately 1.5 hours in total.Â
During this surgery, skin excision and tissue excision was not required. This is because treatment plan focused on removing excessive fatty tissues, and additional skin removal was not necessary to achieve required body contour.Â
Since we avoided tissue excision, there was no need to perform larger incision, and this leads to minimal visible scarring. Please note, this decision of doing liposuction only and not performing tissue excision varies patient to patient. Some patients may require direct excision of glandular tissue or excess skin.
Conclusion
This case highlights the treatment of bilateral accessory breast tissue in a 38-year-old woman from Kandivali, Mumbai, who had experienced underarm fullness for approximately 3–4 years following pregnancy.
Dr. Vishal Patel performed accessory breast removal using liposuction without skin excision, with the procedure taking approximately 1.5 hours.
